Welcome to the GlaxoSmithKline IMPACT Awards online application pages. Please read the guidelines for the awards before starting to fill in the application form. This is an online application form, which may be slightly different to other application forms you have used.

There are help screens next to some of the questions to help you with your answers, or to remind you of the criteria for these awards.

When you have completed the form you will be asked to submit it and it will be stored at The King's Fund. You will be sent an email to confirm the form has been received, attaching a copy of your application. If you do not receive this email please email us as it may mean that we have not received your application.

Referees

In the form we ask for the names and addresses of two referees. We will only contact them if you are shortlisted and they will be sent a two page form to complete. If you would like to view the referees questions and the rules on who can be a referee please click here.

Accounts

You will be asked to provide your most recent accounts. We do not accept draft accounts, and the accounts should be audited or independently examined and have been produced within 10 months of your year-end. You can either attach your accounts electronically to your on-line application form, post them to The King's Fund, or confirm that they are available on-line at the Charity Commission (this last option is currently only available to charities registered with the Charity Commission for England and Wales). You will be given instructions on how to submit your accounts. Whichever option you choose you must ensure that we receive your accounts by the application deadline of 5pm on Friday 26th September 2008 or we will not be able to consider your application for an IMPACT Award.
